Dr. Rakesh Rikhye:

Dr. Rakesh Rikhye graduated from the University of Toronto Medical School. He specialized in Family Medicine in Texas, U.S.A. Dr. Rikhye has been in practice for many years and has accumulated vast knowledge and experience in all aspects of Family Medicine. Dr. Rikhye is fluent in English, Hindi and Punjabi.

Dr. Rajneesh Sikri:

Dr. Rajneesh Sikri studied medicine in erstwhile USSR from Yerewan State Medical University and graduated in 1997.He practiced medicine for 7 years in India before moving to Canada. Dr. Sikri was involved in areas of Clinical research with McMaster University and University of Toronto. He practiced medicine in Nova Scotia for 4 years. He joined CANES FHT in January 2012.

Dr. Wasira Bokore:

Dr. Bokore is originally from Tanzania, East Africa. He graduated from University of Dar-es-Salaam Medical School in 1999. Following his Internship he migrated to the USA for further studies. Dr. Bokore was involved in areas of Clinical Research before joining University of Minnesota in 2006 to pursue Residency in Family Practice. Dr. Bokore moved to Toronto, Canada and joined CANES FHT in 2009. Dr. Bokore is a strong advocate of Quality improvement in Health care and has a special interest in Diabetes, Geriatrics and Cardiovascular diseases. He currently serves as a Lead Physician for CANES FHT.

Dr Maira Obnamia:

Dr. Obnamia studied medicine in the Philippines and graduated in 2003. Following 1 year internship, she moved to Canada and worked in Clinical Research at Toronto General Hospital.  She then pursued Residency in US.  She then started practice by joining CANES in 2011.

Elana Goldin-Lerman, MA, RSW, RMFT

Elana Goldin-Lerman is your Social Worker at CANES Family Health Team. She holds a Masters Degree in Counselling Psychology and has further pursued training in Marriage and Family Therapy. She is registered with the Ontario College of Social Workers and Social Service Workers. She is also registered with the Ontario Association and Canadian Registry for Marriage and Family Therapy. She teaches at Seneca College and holds a private practice as well.

Shahzadi Devje,  RD, CDE, MSc.

Shahzadi Devje is a Registered Dietitian and a Certified Diabetes Educator with extensive experience in community and independent practice across the GTA. She currently practices dietetics counselling in a community and corporate setting and is also a Faculty Member for the Institute for Health Communication. She holds a MSc in Public Health Nutrition, a Post Graduate Diploma in Clinical Dietetics, and a BSc in Nutrition. After completing her first degree, she worked for the Food Standards Agency (FDA equivalent) in London, UK. Shahzadi's unique and rich multi-cultural background enables her to identify with a wide spectrum of people and communicate in several South Asian languages.

Crystal Ward BScN, RN:

Crystal Ward RN has been involved in Community Healthcare since 1999. First as a PSW in Windsor, then she moved back to Brampton to go into Nursing.  Crystal Ward graduated with an Honours Bachelor of Science in Nursing from York University in the Spring of 2007. Crystal continued to provide one-to-one care to patients in their homes with the Victorian Order of Nurses in Brampton. Crystal possesses extensive knowledge in Community Health Nursing from Paediatric nursing to Geriatric Nursing  and from Palliative Care to Chronic Disease Management. Crystal is a passionate patient advocate and believes strongly in quality improvement in our Health Care System. Crystal Joined the CANES Family Health Team in December 2012. Crystal welcomes the opportunity to meet the patients of the CANES FHT and to answer any questions (provide information) that any of you may have regarding your health, medications, disease management or that of your family.  

Michael Levitt, Executive DirectorSince joining the not-for-profit sector in 2007, Michael has led new start-up organizations, and reorganization of organizations seeking expert growth leadership. In the past 7 years, team performance and program reviews have saved over $250,000, without staff reductions. Workflow process improvements between departments have increased productivity and client deliverables without increasing costs.Mr. Levitt brings to the task his experience from a 25-year career in Healthcare, finance and information technology. Michael has served as Director for two Family Health Teams, and as a Director of Finance & Corporate

Services for a large Community Health Centre.  Through community engagement, fund raising, and governmental cooperation, thousands of patients now have a primary care physician. This reduces wait times at Ontario's emergency rooms, by increasing access to primary care, when and where the patient lives.  

A Message From Your Reception Team

As a growing Family Health Team, patients are asked to book an appointment. We are available to take your calls for booking appointments with the Physicians and other Health Professionals from Monday to Thursday from 9:00 AM to 8:00 PM; Friday from 9:30 AM to 4:30 PM and Saturday from 9:00AM to 1:00 PM.

We appreciate your patience while we put you on hold and we understand that it is quite frustrating getting in touch with our receptionist team through telephone. We are working to improve this aspect of the practice. Adjustments are being made to assist us in serving you more efficiently and in a more timely manner.

Ammara  Naseem, Anjana Dhakal and Nikhila Veluri: The receptionist team wants to express this welcome message for our CANES FHT members. We have really enjoyed working as part of the CANES Family Health Team to help our patients and contribute our efforts to work as a bridge among the patients and the physicians. Furthermore, we are proud of our patients who have given us their continuous feedback and support to make our service more improved and efficient. You will be greeted by us when you first arrive at the clinic and check-in for your appointment. 

Medical Office Assistants:

Priyanka Natalia and Shelly Levesque: After you are greeted by the reception team, you will be greeted by us before you see the doctor. We perform clinical tasks to support the work of physicians and other health professionals. Our routine tasks are preparing patient for examinations, measuring vital signs, recording patient’s information, preparing and handling medical instruments, supplies and specimens. We explain treatment procedures and physician instructions to patients. We make your visit in the office run smoothly. We also contact medical facilities or departments to schedule patients for tests and/or specialists.
