Reconnect includes Adult Day Services, Case Management, Community Programs, Home Help and Personal Care, Meals on Wheels, Older Adult Centre, Respite Care, Supportive Housing, Therapeutic Falls Prevention and Exercise, and Transportation. CANES partners with Reconnect through our Treat at Home Mental Health program, providing in-home care to those who may need counselling from a Psychogeriatric Nurse.
